- Complete the short AQUA Registry Interest Form.
- The AQUA Registry team will review the interest form to make sure the practice meets all requirements to proceed.
- Upon interest form approval, the practice will receive an email containing a link to our online sign up portal.
- The practice must then access the sign up portal to review and sign the required participation documents and submit provider information.
The AQUA Registry offers three subscription levels that provide practices with a variety of options to fit their needs and budgets to ensure that all practices wishing to join the only national urology–specialty-wide registry can do so.

All subscribers who are not current AQUA Registry participants are subject to an installation fee of $500 per practice for practices with at least one AUA member and $1,000 for practices without AUA members. Pricing varies by subscription level and practice size.
